BOTOX Ⓡ Cosmetic

Botox injections are a popular non-surgical option to reduce or eliminate frown lines throughout the face. Common treatment areas include forehead creases, between the eyebrows, around the eyes (crows feet), bands in the neck. This medication can also help relieve migraines, excess sweating, and vasospasm in the hands.

Soft tissue fillers

Fillers are can be used to correct signs of aging by restoring facial volume that has been lost over time. There are many types of soft tissue fillers that can be injected under the skin at different levels to help achieve this goal. A detailed approach and plan are required before each session. Common treatment areas include the forehead, cheek, tear troughs, and jowls.

Facial Aesthetic Surgical Options

Eyelid Surgery

As we age, the structures around the eyes begin to change as well. The skin around the upper and lower eyelids lose their elasticity, the muscles can weaken/stretch, and the fat pads begin to shift. This can cause different areas of the eye to develop more wrinkles and appear more droopy (ptotic) and puffy in unwanted areas. Restoring or enhancing these eyelid structures can be achieved  through a number of different procedures.

Upper Blepharoplasty

This procedure involves removing excess skin, fat, and/or muscle from the upper eyelids to improve both appearance and vision. An upper blepharoplasty can lead to a more youthful and refreshed appearance, enhancing the overall look of the eyes.

Lower Blepharoplasty

The skin, fat, and/or muscle from the lower eyelids lose their elasticity, tone, and structural support over time. This procedure involves removing and/or re-distributing these structures to improve both the contour and shape of the lower eyelid.

Ptosis repair

As we age, the muscles that lift the eyelids (levator muscle and superior tarsal muscle) weaken and stretch along with the surrounding soft tissu. A ptosis repair can be done to correct the droopy eyelid by repositioning and shortening the levator muscle

Asian eyelid surgery

The following are general key features of an upper Asian eyelid that make it distinct from a Caucasian eyelid (1):

  • The upper lid has more fullness

  • The palpebral fissure is narrower

  • Upward lateral canthal tilt

  • Presence of an epicanthal fold

  • Wider intercanthal distance

Each individual is unique of course and these morphologies have been further categorized and  described to help surgeons and patients achieve their shared aesthetic goals. The following procedures can be done to enhance the appearance of the Asian eyelid:

  • Double fold blepharoplasty

  • Epicanthoplasty

  • Sub-brow excision

  • +/- ptosis repair

  • +/- lower blepharoplasty

Double fold blepharoplasty

This procedure is also known as double eyelid surgery or Asian blepharoplasty. It is a common cosmetic procedure that is done to create a visible supratarsal crease in the upper eyelid above the eyelashes, allowing the eyes to appear larger, more open, and potentially more expressive.

Epicanthoplasty

This is a cosmetic surgical procedure that modifies the inner corner of the eye known as the epicanthal fold, creating a more open, wider, more defined and balanced appearance. It is often performed in conjunction with double fold blepharoplasty.

Sub-brow excision

The outer aspect of the upper eyelid may be in excess causing lateral hooding. This can be addressed effectively with a sub-brow excision. Skin and fat are removed from under the brow to enhance the appearance and shape of the eye.

Brow Procedures

In general, the soft tissue around the brow descends over time, contributing to hooding of the upper eyelid and rhytids (or wrinkles) start to appear.

There are key differences between the aesthetic female and male brow.

Unlike other areas of the face where the soft tissues lose elasticity and droop, there is paradoxical elevation of the eyebrow as women age. The apex of the youthful brow (the outer aspect) is lost over time as the inner and middle brow elevates, creating a flattened appearance (1).

In men, the brow is naturally flatter and straighter, situated at or slightly below the bony orbital rim.

Forehead rejuvenation can be achieved via various access points (e.g. endoscopic, hairline, direct brow or browplasty, etc.). These procedures can help recreate the aesthetic shape of the brow, elevate the descended brow, help correct eyelid hooding, and address deep forehead wrinkles.

Facelift

There are many, complex changes that occur at the bony and different soft tissue areas of the face, resulting in characteristic changes as we age. In our early to mid adult years, forehead wrinkles, frown lines and crow’s feet start to appear, and the nasolabial fold deepens. Vertical lines around the mouth develop and by the 5th to 6th decade, wrinkling of the perioral region and neck become more obvious. The soft tissue loses its structure and the fat in different compartments of our face start to shift, creating jowls and marionette lines in the lower face (4).

The overall goal of facelift surgery (also known as rhytidectomy) is to rejuvenate the middle and lower face by addressing the natural descent of the soft tissue structures noted above. It involves tightening the facial soft tissue structures and removing and re-draping the facial skin to achieve a smoother appearance. Often this procedure is combined with a necklift.

Neck Rejuvenation

As we age, the skin around the neck develops more wrinkles, laxity, and droop. The underlying supportive soft tissue structures (e.g. the deep retaining ligaments) also start to lose their tone elasticity along with the platysma muscle. This causes the inner edges of the muscle to descend and move outwardly leading to the formation of vertical neck platysmal bands. Fat in between and under the muscle can also herniate.

The goal of neck rejuvenation surgery is to re-contour the shape of the neck by removing excess fat on top of or below the platysmal muscle, and by re-suspending, plicating the inner edges of the muscle, or removing redundant muscle when necessary (5).

Submental liposuction

This procedure involves removing excess fat that lies on top of the platysmal muscle using liposuction. This procedure can be effective at reshaping the contour of the neck, creating more definition between the jaw and neck.

Mini neck lift

Combined with submental liposuction, an incision is made in the double chin crease fold such that the fat under the platysmal muscle can be removed. Following this, the platysmal muscle can then be tightened during the same procedure. This procedure addresses the fullness created by the fat underlying the neck muscle.

Standard neck lift

This cosmetic procedure involves removing the extra skin of the neck and the excess fat around the jowls and fat above/below the neck muscle. Following this, the skin is re-draped to tighten and create more definition of the neck. In doing so, the neck bands of will be corrected.
